  This form is for a request to withhold directory information until the date the respective student graduates or otherwise leaves Indian Prairie School District. Opt-out preferences may be updated at any time by submitting a new form.

Vendors interested in providing information and materials for Technology Services should complete the provided form. Materials submitted through this form will not be considered for RFP's, as the process guiding an RFP process have explicit requirements. Your information will be stored in our vendor database and may be referenced when generating a vendor list for quotes/proposals.
